- جهاز مقوم نظم القلب مزيل الرجفان القابل للزرع أو بالمختصر ICD أو AICD (بالإنجليزية: automated implantable cardioverter defibrillator) هو جهاز يتم زراعته داخل الجسم وقادر على أداء كل من تقويم نظم القلب بالصدمة، وتقويم الرجفان البطيني للقلب. الجهاز بالتالي قادر على تصحيح معظم حالات عدم انتظام ضربات القلب التي تهدد الحياة بسبب قلبي. يعتبر هذا الجهاز الخط الأول في العلاج والوقاية للمرضى المعرضين لخطر الموت المفاجئ بسبب الرجفان البطيني . الأجهزة الحالية يمكن برمجتها للكشف عن عدم انتظام ضربات القلب وتقديم العلاج عن طريق برمجة سرعة القلب منخفضة الطاقة والطاقة العالية بالصدمات. إن مصطلح "AICD" كدلالة على الجهاز هو أيضاً علامة تجارية "Boston Scientific corporation"، وبالتالي فإن الاختصار "ICD" أفضل كمصطلح للاستعمال والتداول .بطاريات الجهاز الحالي تدوم حوالي 6-10 سنوات، وبعد ذلك يحتاج الجهاز للاستبدال بأجهزة بتكنولوجيا أقوى (بطاريات مع قدرة أكبر أو ربما في المستقبل مع البطاريات القابلة لإعادة الشحن) قد يكون من الممكن زيادة عمر البطارية حتى 10 سنوات، والرصاص المستخدم في الجهاز (السلك الكهربائي الموصل للجهاز إلى القلب) لديه متوسط عمر أكبر ولكن يحصل له حالات خلل مختلفة خاصةً في خلل العزل على وجه التحديد أو كسر في موصل مما يتطلب استبداله . - Ein implantierbarer Kardioverter/Defibrillator (ICD, frühere Bezeichnung AICD, von [automatic] implantable cardioverter-defibrillator) ist ein miniaturisiertes automatisches Elektrogerät, das Patienten mit hohem Risiko für lebensbedrohliche Herzrhythmusstörungen mittels einer Operation eingesetzt wird. Die Elektroden des ICD liegen in der Herzkammer und haben dadurch direkten Kontakt zum Herzmuskel. Bei Herzkammerflattern oder -flimmern wird automatisch ein elektrischer Impuls ausgelöst. Durch diesen Stromstoß normalisiert sich die Herzmuskelaktivität und damit die lebenswichtige Pumpleistung des Organs. Das Gerät wird wie ein Herzschrittmacher implantiert. Eine Weiterentwicklung des ICD ist der (S-ICD). ICD werden in Deutschland pro Jahr mit einer Häufigkeit von ca. 270 Implantationen pro einer Million Einwohner neu eingesetzt (Stand: 2019). Diese Zahl wird als europa- sowie als wahrscheinlich weltweit führend angenommen. - An implantable cardioverter-defibrillator (ICD) or automated implantable cardioverter defibrillator (AICD) is a device implantable inside the body, able to perform defibrillation, and depending on the type, cardioversion and pacing of the heart. The ICD is the first-line treatment and prophylactic therapy for patients at risk for sudden cardiac death due to ventricular fibrillation and ventricular tachycardia. "AICD" was trademarked by the Boston Scientific corporation, so the more generic "ICD" is preferred terminology. Current device batteries last about six to ten years; with advances in technology (batteries with more capacity or potentially rechargeable batteries) it may be possible to increase this well past ten years. The lead (the electrical cable connecting the device to the heart) has a much longer average longevity but can incur various types of malfunction, specifically insulation failure or fracture of the conductor, and require replacement. The process of implantation of an ICD system is similar to implantation of a pacemaker. In fact, ICDs are composed of an ICD generator and of wires. The first component or generator contains a computer chip or circuitry with RAM (memory), programmable software, a capacitor and a battery; this is implanted typically under the skin in the left upper chest. The second part of the system is an electrode wire or wires that, similar to pacemakers, are connected to the generator and passed through a vein to the right chambers of the heart. The lead usually lodges in the apex or septum of the right ventricle. Just like pacemakers, ICDs can have a single wire or lead in the heart (in the right ventricle, single chamber ICD), two leads (in the right atrium and right ventricle, dual chamber ICD) or three leads (biventricular ICD, one in the right atrium, one in the right ventricle and one on the outer wall of the left ventricle). The difference between pacemakers and ICDs is that pacemakers are also available as temporary units and are generally designed to correct slow heart rates, i.e. bradycardia, while ICDs are often permanent safeguards against sudden life-threatening arrhythmias. Recent developments include the subcutaneous ICD (S-ICD) which is placed entirely under the skin, leaving the vessels and heart untouched. (en)

ICD, implantable cardioverter defibrillator) – wszczepialne urządzenie medyczne, mające za zadanie przerwać potencjalnie groźną dla życia arytmię i przywrócić rytm zatokowy serca. Wszczepienia urządzenia dokonuje się u chorych, u których pomimo leczenia farmakologicznego istnieje duże ryzyko wystąpienia groźnych dla życia (tak zwanych złośliwych) zaburzeń rytmu serca, mogących doprowadzić do nagłego zatrzymania krążenia i śmierci sercowej. Wskazaniami do tego typu leczenia są:
* przebyte zatrzymania krążenia w mechanizmie migotania komór
* częstoskurcz komorowy niestabilny hemodynamicznie (przebiegający z zaburzeniami krążenia)
* obecnie coraz częściej wskazania do wszczepienia ICD są rozszerzane o przypadki, o których wiadomo, że mogą doprowadzić do zaburzeń rytmu w powyższych mechanizmach. Należą do nich między innymi zespół Brugadów, zespół długiego QT, niewydolność serca z niską frakcją wyrzutową lub kardiomiopatię przerostową.
