- La incisió i drenatge són procediments quirúrgics per alliberar el pus d'una zona o òrgan, habitualment en forma d'abscés, mitjançant un tall (incisió) a través del qual el pus pugui sortir (drenatge). En alguns casos s'utilitza aquesta tècnica per evacuar hematomes importants. El més habitual és la incisió i drenatge d'un abscés cutani (de sota la pell), es parla aleshores de procediment quirúrgic menor. Es realitza tractant la zona amb un antisèptic, i després fent una petita incisió (tall) punxant la pell mitjançant un instrument estèril com una agulla afilada o un bisturí punxegut. Els abscessos cutanis no complicats no necessiten antibiòtics després d'un drenatge reeixit. La bona pràctica mèdica per als abscessos abdominals grans requereix la inserció d'un tub de drenatge, precedit per la inserció d'una via central per si convingués un tractament d'un possible xoc sèptic. - Als Abszessdrainage bezeichnet man in der Medizin eine Methode zur Entlastung eines Abszesses. Eine seit Jahrhunderten bewährte Regel bei der Behandlung von Abszessen lautet Ubi pus, ibi evacua ("Wo sich Eiter findet, dort lasse ihn ab"). In der Regel werden Abszesse chirurgisch eröffnet, mit einer Lasche versorgt, gespült und man wartet eine Sekundärheilung ab. Als Lasche kann hierbei ein Gummischlauch oder ein Teil eines sterilen Handschuhs verwendet werden. Wenn ein Abszess mittels gezielter Punktion oder Miniinzision leicht erreichbar und ein Spülschlauch oder eine Lasche platzierbar ist, kann mit geduldiger, mehrtägiger Spülung mit isotonischer Kochsalzlösung und gleichzeitiger systemischer Gabe von Antibiotika ein Abheilen des Abszesses erreicht werden, ohne dass der Abszess breit eröffnet werden muss. Insbesondere bei postoperativen Flüssigkeitsansammlungen im Bauchraum hat sich diese Methode bewährt, da sie dem Patienten oft eine erneute OP erspart. Dieser Artikel behandelt ein Gesundheitsthema. - Incision and drainage (I&D), also known as clinical lancing, are minor surgical procedures to release pus or pressure built up under the skin, such as from an abscess, boil, or infected paranasal sinus. It is performed by treating the area with an antiseptic, such as iodine-based solution, and then making a small incision to puncture the skin using a sterile instrument such as a sharp needle or a pointed scalpel. This allows the pus fluid to escape by draining out through the incision. Good medical practice for large abdominal abscesses requires insertion of a drainage tube, preceded by insertion of a PICC line to enable readiness of treatment for possible septic shock. (en)
