Les limitations du moteur précédent en Flash ont poussé McMillen à repenser le jeu de zéro et à sortir cette version comme étant un nouveau jeu. Le second objectif pour McMillen était de changer l'aspect graphique du jeu, pour opter pour du pixel-art tel qu'il l'avait souhaité dès le départ. Unlike the game's predecessor, Rebirth has a limited multiplayer mode, allowing an additional player in Rebirth, later increased to three additional players in Afterbirth and Afterbirth+. Full co-op support was added to Repentance, where up to four players are able to play as any of the playable characters. Rebirth released to critical acclaim. Reviewers praised its gameplay and improvements compared to the original Binding of Isaac, but criticized its graphic imagery.
