- Ein Bruchband ist ein medizinisches Hilfsmittel, das am ehesten mit einer Korsage verglichen werden kann. Es dient dazu, einen Leistenbruch oder Nabelbruch am Prolabieren (Heraustreten) zu hindern, und soll die darunterliegende Bauchdecke so weit stabilisieren, dass eine Besserung eintritt, beziehungsweise der Patient beschwerdefrei damit leben kann. Dazu bestand das Bruchband in früheren Zeiten meistens aus einem straffen Ledergürtel, manchmal auch mit Gummizug, an dem die Pelotte, eine ursprünglich aus Holz, später aus Metall gefertigte Platte, angebracht wurde. Diese wurde auf die Bruchpforte gesetzt und sollte den Bruch zurückdrängen. Heutzutage wird die Prozedur in dieser Form nicht mehr angewandt, da der Nutzen gering war, das Schadpotential hingegen sehr hoch. Bei Männern kam es häufig unter dem Einfluss des Bruchbandes zu einer Hodenatrophie mit Verödung des jeweiligen Hodens. Auch schädigte der ständig anliegende Druck das Gewebe und führt zu Hautulzerationen, bis die Hernie durch die Haut brach. Heute stehen schonende Bruchbänder und Pelotten aus elastischem Kunststoff zur Verfügung, die per Klettband miteinander verbunden werden. Obwohl sie ohne Rezept auch online in verschiedenen Größen bestellt werden können, bleiben sie umstritten. Vor Anwendung empfiehlt sich im Zweifel ein Besuch beim Arzt und professionelle Anpassung zum Beispiel in einem Sanitätshaus. Therapie der Wahl beim Leistenbruch ist die alsbaldige Operation. Hierfür stehen neben den klassischen offenen Verfahren (Operation nach Bassini und ihre Variationen, Operation nach Shouldice oder ) auch offene und minimalinvasive Verfahren mit Verstärkung der Leiste durch ein Polypropylen-Netz (offen: Lichtenstein-Operation; minimalinvasiv: TAPP oder TEP) zur Verfügung. Bruchbänder wurden bereits sehr früh zur Linderung dieses Krankheitsbildes entwickelt. Aus archäologischen Ausgrabungen lagen bis 1992 22 Funde von Bruchbändern in Mitteleuropa vor, die nahezu alle aus der Merowingerzeit stammen. Noch im 19. Jahrhundert wurden (verbesserte) Bruchbänder häufig als Alternative zur nicht ohne Grund gefürchteten Radikaloperation von Bauchwandbrüchen benutzt. (de)
- In medicine, a truss is a kind of surgical appliance, particularly one used for hernia patients. A truss provides support for the herniated area, using a pad and belt arrangement to hold it in the correct position, just when it is put on before moving from bed. Of historical interest, a variety of trusses are listed in the Snowden & Brother's catalog of the American Civil War era. Early versions of the hernia truss were daunting contraptions made from leather and steel with metal springs. The 19th century Eggleston's Truss from Chicago was described as follows: "Eggleston's Truss has a pad different from all others. It is cup-shaped, with a self-adjusting ball in the centre, and adapts itself to all positions of the body, while the ball in the cup presses back the intestines just as a person does with the finger. With light pressure the hernia is held securely day and night, and a radical cure is certain. It is easy, durable and cheap." Later developments resulted in the Cluthe truss, described in Cluthe's Advice to the Ruptured, first published in 1912. This book also describes dozens of hernia cases which appeared to be cured by this relatively primitive truss. Such testimonials are no longer considered to be scientific evidence. Currently most doctors and surgeons do not prescribe trusses. Even some of today's trusses use metal springs to apply pressure to the hernia, via a pad which can be quite hard, and usually bulges into the hernia. This inward bulging prevents the edges of the hernia from coming together and could, in theory, enlarge the hernia. Many trusses also do not fit well. It is difficult to design a truss which keeps the pad permanently in contact with the hernia. Unless a truss can achieve this purpose, the hernia may continue to enlarge. External devices such as a truss to maintain reduction of the hernia without repairing the underlying defect can be used to avoid surgical risks, mainly chronic pain risk. (en)

I cuscinetti si appoggiano nel punto in cui si verifica la fuoriuscita dell'ernia e la contengono. Se ne consiglia l'uso nel caso in cui il paziente non sia in condizioni di subire un intervento chirurgico in brevi termini. Il cinto erniario non ha una funzione terapeutica ma soltanto palliativa. La continua compressione è certamente dannosa e a lungo andare potrebbe verificarsi la formazione di aderenze che potrebbero compromettere un eventuale intervento. (it)
