- Response Evaluation Criteria In Solid Tumors (RECIST), deutsch etwa Kriterien für die Bewertung des Ansprechens der Behandlung bei soliden Tumoren, ist eine Sammlung von veröffentlichten Regeln, die die Bewertung der Behandlungserfolge von Krebserkrankungen in medizinischen Forschungsstudien objektivieren sollen. Die Regeln definieren exakt, wann eine Krebserkrankung unter Therapie zurückgeht („anspricht“), unverändert (stabil) bleibt oder fortschreitet (Progression). Die Kriterien wurden im Februar 2000 durch eine internationale Arbeitsgruppe publiziert, der u. a. die EORTC und die Studiengruppen der nationalen Krebsinstitute in den USA und sowie die in den Niederlanden ansässige angehörten. Die aktuelle Version 1.1 stammt vom Januar 2009. Die meisten klinischen Studien zu Krebstherapien verwenden mittlerweile RECIST. - Response evaluation criteria in solid tumors (RECIST) is a set of published rules that define when tumors in cancer patients improve ("respond"), stay the same ("stabilize"), or worsen ("progress") during treatment. The criteria were published in February 2000 by an international collaboration including the European Organisation for Research and Treatment of Cancer (EORTC), National Cancer Institute of the United States, and the Clinical Trials Group. Today, the majority of clinical trials evaluating cancer treatments for objective response in solid tumors use RECIST. These criteria were developed and published in February 2000, and subsequently updated in 2009. The criteria are specifically not meant to determine whether patients have improved or not, as these are tumor-centric, not patient centric criteria. This distinction must be made by both the treating physicians and the cancer patients themselves. Many oncologists in their daily clinical practice follow their patients' malignant disease by means of repeated imaging studies and make decisions about continuing therapy on the basis of both objective and symptomatic criteria. It is not intended that these RECIST guidelines play a role in that decision making, except if determined appropriate by the treating oncologist. (en)
- I criteri RECIST (Response Evaluation Criteria in Solid Tumors) sono un sistema creato dal RECIST Working Group utilizzato in radiologia oncologica per la stima della risposta ai trattamenti citostatici (chemioterapia antitumorale e radioterapia). Tali criteri sono primariamente dimensionali, applicabili indistintamente a tutte le metodiche radiologiche (tranne l'ecografia) e consentono di determinare se un paziente ha avuto una risposta alle terapie completa o parziale oppure se la sua malattia è stabile o è andata in progressione. L'ultima versione dei criteri RECIST è la 1.1 (RECIST 1.1). Nel determinare la risposta devono essere utilizzate massimo 5 lesioni (dette lesioni target, non più di 2 per organo) rappresentative della malattia del paziente. I linfonodi per essere misurabili devono avere una lunghezza dell'asse corto di almeno 15 mm ed in generale lesioni più piccole di un centimetro non sono da tenere in considerazione (linfonodi di dimensioni inferiori al centimetro sono da considerare non patologici). Le metastasi ossee con componente di tessuto molle di diametro inferiore al centimetro non vanno considerate per via della loro tendenza a calcificare ed ingrandirsi anche in assenza di progressione, mentre cautela va esercitata nella misurazione di lesioni con componenti liquide o cistiche. In alcuni casi è necessaria un'attenta diagnosi differenziale per escludere lesioni non di pertinenza oncologica dal conteggio. Le lesioni di carcinosi peritoneale o pleuriche sono difficilmente misurabili. La Tomografia a emissione di positroni può anch'essa essere utilizzata per lo studio della risposta alle terapie citostatiche.
