- التهاب الجلد حول الفم.( بالإنجليزية Perioral dermatitis). هو مرض التهابي يتظاهر بحمامى وحطاطات وبثرات حول الفم ,وتترافق الإصابة بحس وخز وحكة أو حرق.المحيط بالفم وكثيراً ما يأثر الجلد حول الأنف أيضاً, وأحياناً كذلك الجلد حول العيون.وهو مرض شائع عند الإناث بين عمر 15-25 سنة ونادرا ما يصيب الذكور. - La dermatitis perioral, també coneguda com a dermatitis periorificial, és un tipus comú d'erupció cutània. Els símptomes inclouen múltiples pàpules i butllofes petites (1-2 mm) de vegades amb enrogiment i escates de fons, localitzats a la pell al voltant de la boca i les fosses nasals. Amb menys freqüència, els ulls i els genitals poden estar afectats. Pot ser persistent o recurrent i s'assembla especialment a la rosàcia i fins a cert punt a l'acne i la dermatitis al·lèrgica. El terme "dermatitis" és un nom inadequat perquè no es tracta d'un procés èczematós. La causa no està clara. Els esteroides tòpics s'associen amb la malaltia i les cremes hidratants i cosmètiques poden contribuir. El mecanisme subjacent pot implicar un bloqueig de la superfície de la pell seguit d'un creixement excessiu posterior de la flora de la pell. La pasta de dents fluorada i alguns microorganismes, inclosa la Candida, també poden empitjorar la malaltia, però el seu paper en aquesta afecció no està clar. Es considera una malaltia del fol·licle pilós amb mostres de biòpsia que mostren canvis microscòpics al voltant del fol·licle. El diagnòstic es basa en els símptomes. El tractament sol ser aturant els esteroides tòpics, canviant cosmètics i, en casos més greus, prenent tetraciclines per via oral. L'aturada dels esteroides pot empitjorar inicialment l'erupció. S'estima que la malaltia afecta el 0,5-1% de les persones a l'any al món desenvolupat. - Perioral dermatitis, also known as periorificial dermatitis, is a common type of skin rash. Symptoms include multiple small (1–2 mm) bumps and blisters sometimes with background redness and scale, localized to the skin around the mouth and nostrils. Less commonly the eyes and genitalia may be involved. It can be persistent or recurring and resembles particularly rosacea and to some extent acne and allergic dermatitis. The term "dermatitis" is a misnomer because this is not an eczematous process. The cause is unclear. Topical steroids are associated with the condition and moisturizers and cosmetics may contribute. The underlying mechanism may involve blockage of the skin surface followed by subsequent excessive growth of skin flora. Fluoridated toothpaste and some micro-organisms including Candida may also worsen the condition, but their roles in this condition are unclear. It is considered a disease of the hair follicle with biopsy samples showing microscopic changes around the hair follicle. Diagnosis is based on symptoms. Treatment is typically by stopping topical steroids, changing cosmetics, and in more severe cases, taking tetracyclines by mouth. Stopping steroids may initially worsen the rash. The condition is estimated to affect 0.5-1% of people a year in the developed world. Up to 90% of those affected are women between the ages of 16 and 45 years, though it also affects children and the elderly, and has an increasing incidence in men. (en)
- Clownseczeem of dermatitis perioralis is een algemeen voorkomende huidaandoening rond de mond die gepaard gaat met rode, jeukende plekken van enkele millimeters groot. De direct aan het lippenrood grenzende huid is vaak niet aangedaan. Soms geven de afwijkingen een brandende of stekende pijn. Vaak vertoont de huid ook enige schilfering. Behalve rond de mond komen de afwijkingen ook voor aan weerszijden van de neus en rond de ogen. Vooral mensen met een vet huidtype hebben een grotere kans op het ontwikkelen van clownseczeem, vooral als er regelmatig cosmetica op de gezichtshuid worden gebruikt. Vrouwen hebben vaker last van clownseczeem dan mannen. De klachten kunnen worden uitgelokt door het smeren van cosmetica in het gebied rond de mond, zoals dag- en nachtcremes. Afsluiten van de poriën in de huid speelt hierbij een rol. Er is een duidelijke relatie tussen het smeren van hormooncremes in het gelaat en het ontstaan van clownseczeem. Wanneer de arts de verschijnselen van clownseczeem bij vergissing aanziet voor 'gewoon' eczeem en hiervoor nog sterker hormooncremes voorschrijft, kunnen de klachten verder verergeren. (nl)
