Il gioco è ispirato a Rocky Joe, un manga scritto e disegnato da Tetsuya Chiba ed Asao Takamori negli anni settanta, che era stato anche adattato in una serie anime. Il giocatore controlla il personaggio di Joe Yabuki, un aspirante boxer, nel suo percorso che lo porterà a diventare da ragazzino di strada a campione del mondo. Il gioco utilizza un tradizionale sistema di gioco picchiaduro, con alcune porzioni di gioco ambientate all'interno di un ring ed altre in alcuni ambienti esterni come strade o vicoli.
