Cardiac resynchronisation therapy (CRT or CRT-P) is the insertion of electrodes in the left and right ventricles of the heart, as well as on occasion the right atrium, to treat heart failure by coordinating the function of the left and right ventricles via a pacemaker, a small device inserted into the interior chest wall. CRT is indicated in patients with a low ejection fraction (typically <35%) indicating heart failure, where electrical activity has been compromised, with prolonged QRS duration to >120 ms.

The insertion of electrodes into the ventricles is done under local anesthetic, with access to the ventricles most commonly via the subclavian vein, although access may be conferred from the axillary or cephalic veins. Right ventricular access is direct, while left ventricular access is conferred via the coronary sinus (CS). CRT defibrillators (CRT-D) also incorporate the additional function of an implantable cardioverter-defibrillator (ICD), to quickly terminate an abnormally fast, life-threatening heart rhythm. CRT and CRT-D have become increasingly important therapeutic options for patients with moderate and severe heart failure. W efekcie dochodzi do zmniejszenia objętości wyrzutowej. Zastosowanie terapii resynchronizującej u chorych na zaawansowaną niewydolność serca ma korzystny wpływ na klasę NYHA, wydolność fizyczną i jakość życia. Wytyczne Europejskiego Towarzystwa Kardiologicznego (ESC) z 2007 roku potwierdzają wartość dwukomorowej terapii resynchronizującej w leczeniu chorych z zaawansowaną niewydolnością serca (klasa NYHA III/IV) w celu zmniejszenia objawów chorobowych i śmiertelności. Em alguns casos, também é possível estimular apenas o ventrículo esquerdo. Na insuficiência cardíaca avançada, esse implante é utilizado para tratar um distúrbio da estimulação do ventrículo esquerdo (como dessincronia na contração). No eletrocardiograma, este fenômeno é acompanhado por ampliação do complexo QRS, sendo a origem de tal modificação, em muitos casos, o bloqueio do ramo esquerdo do feixe. Outros nomes: estimulação de ressincronização, a estimulação de dupla câmara A assincronia sistólica entre os ventrículos esquerdo e o direito leva à contração antecipada do septo em relação à parede lateral do ventrículo esquerdo, o que dificulta o enchimento durante a diástole. Sendo assim, tal assincronia diminui o volume de ejeção sistólico. O uso de terapia de ressincronização cardíaca em pacientes com insuficiência cardíaca avançada melhora qualidade de vida dos pacientes e diminui o grau de severidade da doença (medido pela classificação da New York Heart Association). Uma diretriz de 2007 da Sociedade Europeia de Cardiologia (ESC) reforça o valor da terapia de ressincronização cardíaca biventricular para tratamento de pacientes com insuficiência cardíaca avançada (classe NYHA III / IV), com redução dos sintomas e da mortalidade.
