La malaltia és habitualment causada per una hemòlisi excessiva, en la qual es destrueixen un gran nombre de glòbuls vermells, alliberant així l'hemoglobina al plasma. - La hemoglobinurio estas la ĉeesto de libera hemoglobino en la urino kiel konsekvenco de intravaskula hemolizo. La hemoglobino estas molekulo kiu estas aligita al la eritrocitoj kaj helpas en la transporto de oksigeno kaj de karbona dioksido tra la korpo. - Als Hämoglobinurie wird eine Ausscheidung von Hämoglobin (dem roten Blutfarbstoff) über die Nieren bezeichnet, die bei starker Ausprägung zu einem akuten Nierenversagen führen kann. Eine Hämoglobinurie tritt erst auf, wenn aufgrund einer massiven Hämolyse die des von der Leber gebildeten Haptoglobins erschöpft ist oder bei einer durch einen Leberschaden verminderten Haptoglobinkonzentration im Plasma gleichzeitig die tubuläre Resorptionskapazität der Nieren erschöpft ist. Mit der Ausscheidung des Hämoglobins im Urin verfärbt sich dieser dunkel. Die Ursache besteht in einer akuten, meist massiven Hämolyse, wie sie bei verschiedenen hämolytischen Anämien, Transfusionszwischenfällen, Malaria oder Autoantikörperkrankheiten (siehe Kälteagglutinine) auftreten kann. Allerdings kann erst die weitere Abklärung (Anamnese, andere Laborparameter, Symptome, Verlaufskurve der Hämoglobin-Ausscheidung: andauernd oder intermittierend bzw. paroxysmal) im gegebenen Fall vom Einzelbefund einer Hämoglobinurie zur Diagnose führen, die letztlich auch lauten kann:

Hemoglobine is een eiwit dat in rode bloedcellen voorkomt en nergens anders. Bij bloedverlies in de urinewegen kunnen rode bloedcellen in de urine voorkomen, dit heet hematurie. Bij hemoglobinurie zijn er met de microscoop geen rode bloedcellen in de urine te zien, terwijl er wel hemoglobine in de urine zit, die kleurt dus wel rood. Dit kan optreden als de rode bloedcellen in de urine gezeten hebben, maar zijn stukgegaan, bijvoorbeeld door osmose doordat de urine sterk verdund is, in dat geval is de betekenis dezelfde als van hematurie. Het is echter ook mogelijk dat rode bloedcellen al in de bloedbaan kapot zijn gegaan (hemolyse), zodat vrij hemoglobine (als eiwit) door de nier wordt uitgescheiden.
