A cochlear implant (CI) is a surgically implanted neuroprosthesis that provides a person who has moderate-to-profound sensorineural hearing loss with sound perception. With the help of therapy, cochlear implants may allow for improved speech understanding in both quiet and noisy environments.   • Das Cochlea-Implantat (englisch cochlear implant, CI) ist eine Hörprothese für Gehörlose und Ertaubte, deren Hörnerv als Teilorgan der auditiven Wahrnehmung noch funktionsfähig ist, sowie für hochgradig Schwerhörige, bei denen die Versorgung mit einem Hörgerät nicht mehr ausreichend ist. Das CI-System besteht einerseits aus einem Sprachprozessor mit einem Mikrofon, an dem eine Sendespule mit Magnet angebunden ist. Andererseits besteht es aus dem eigentlichen Implantat, das sich aus einem weiteren Magneten, einer Empfangsspule, dem Stimulator und dem Elektrodenträger mit den Stimulationselektroden zusammensetzt. Die Elektroden werden in die Cochlea (Hörschnecke) eingeführt. Die Empfangsspule wird im Schädelknochen nahe der Ohrmuschel unter der Haut platziert. Die Sendespule des Prozessors haftet mit Hilfe der Magneten auf der Kopfhaut über der Empfangsspule des Implantats. Die Spannungsversorgung des Implantats erfolgt durch die Kopfhaut mittels elektromagnetischer Induktion. Die Signalübertragung erfolgt mit Hochfrequenzwellen. The outside component is generally worn behind the ear, but could also be attached to clothing, for example, in young children. This component, the sound processor, contains microphones, electronics that include digital signal processor (DSP) chips, battery, and a coil that transmits a signal to the implant across the skin. The inside component, the actual implant, has a coil to receive signals, electronics, and an array of electrodes which is placed into the cochlea, which stimulate the cochlear nerve. The surgical procedure is performed under general anesthesia. Surgical risks are minimal and most individuals will undergo outpatient surgery and go home the same day. However, some individuals will experience dizziness, and on rare occasions, tinnitus or facial nerve bruising. From the early days of implants in the 1970s and the 1980s, speech perception via an implant has steadily increased. More than 200,000 people in the United States had received a CI through 2019. Many users of modern implants gain reasonable to good hearing and speech perception skills post-implantation, especially when combined with lipreading. One of the challenges that remain with these implants is that hearing and speech understanding skills after implantation show a wide range of variation across individual implant users. Factors such as age of implantation, parental involvement and education level, duration and cause of hearing loss, how the implant is situated in the cochlea, the overall health of the cochlear nerve, but also individual capabilities of re-learning are considered to contribute to this variation. Implant ślimakowy pozwala na rehabilitację osoby z głębokim niedosłuchem zmysłowo-nerwowym. Pacjent zyskuje nie słuch fizjologiczny, ale słuch do niego jakościowo zbliżony. Całość systemu implantu ślimakowego składa się z dwóch części: wewnętrznej, wszczepianej pod skórę, umieszczanej w loży kostnej przygotowanej przez chirurga w zagłębieniu kości czaszki, oraz części zewnętrznej, przetwarzającej sygnały akustyczne w sygnały elektryczne, przekazywane dalej do części wewnętrznej. Elektroda części wewnętrznej umieszczona w ślimaku bezpośrednio pobudza nerw słuchowy, wywołując wrażenia słuchowe. W Polsce stosowane są implanty firm: , Cochlear, Oticon Medical (Neurelec) oraz . Ett barn som är dövt vid födelsen anses ha bäst förutsättningar att dra nytta av ett cochleaimplantat om det opereras in tidigt.Även vuxna personer CI-opereras med goda resultat. För att ett CI ska fungera behövs två delar, en yttre ljudprocessor och en inopererad del, själva implantatet. Den yttre delen påminner om en vanlig hörapparat men är förutom mikrofon försedd med en sändarspole som för över signalerna till implantatet. Den inre delen innehåller en elektrodrad som läggs i hörselsnäckan (cochlean) samt elektronik för att ta emot signalerna från den yttre delen och ge stimulering av hörselnerven. O Implante Coclear possui uma parte externa e outra interna. A parte externa é constituída por um microfone, um microprocessador de fala e um transmissor. A parte interna possui um receptor e estimulador, um eletrodo de referência e um conjunto de eletrodos que são inseridos dentro da cóclea. Esse dispositivo eletrônico tem por objetivo estimular, através desses eletrodos implantados dentro da cóclea, o nervo auditivo que, por sua vez, leva os sinais para o encéfalo onde serão decodificados e interpretados como sons. É válido lembrar que diagnóstico precoce da perda auditiva, ou seja, antes dos 12 meses de vida, é de suma importância, pois possibilita identificar qualquer alteração auditiva num período considerado ideal, período esse em que há maior plasticidade do sistema auditivo. Quanto mais cedo esse processo de diagnóstico e intervenção se iniciar melhor é prognóstico. A demora do diagnóstico acaba refletindo no processo de aquisição e desenvolvimento da linguagem oral das crianças. Após os 3 anos e meio de idade inicia-se um período chamado crítico, que vai até aos 7 anos, onde a plasticidade do sistema auditivo já não é mais a mesma e, por tanto, o sucesso do implante coclear pode variar de criança para criança.
