This information can be found in Tibetan medical books which originated from the 4 Tantras (Root Tantra, Tantra of Enlightenment, Tantra of Instructions, Concluding Tantra) which is also known as rGyudbzhi in Tibetan. To this day, it is the central work upon which Tibetan medicine is based. Tibetan medical theory states that everything in the universe is made up of the 5 proto-elements: sa, chu, me, rLung, and Nam-mkha. But only four play a role in the classification of our illnesses, except Nam-mkha (Space). Each element contains 8 active forces and 17 qualities. Some of these elements are contained in our three bodily energies and their imbalance affects the equilibrium of the three ‘fluids’ (rLung, mkhrispa and badkan). In order words, incense follows a particular ‘lineage’ which can be traced back to the originator. Wierook is een belangrijk element binnen de Tibetaanse cultuur. Naast Tibet zelf wordt het verder voornamelijk gebruikt in Nepal en Bhutan. In de Tibetaanse geneeswijze wordt aan wierook geneeskracht toegedicht. Deze behandelingen staan beschreven in het basiswerk Vier Tantra's, dat bestaat uit de boeken Worteltantra, Tantra van verlichting, Tantra van instructies en Slottantra. Wierook in de Tibetaanse geneeswijze is een onderdeel van de vijf elementen: vuur, aarde, metaal, water en hout. De wierook werd oorspronkelijk voornamelijk in de Tibetaanse kloosters geproduceerd of de medische colleges zoals Mentsikhang op de IJzerberg in Lhasa, waarbij elk soort wierook terug te voeren was naar een bepaalde boeddhistische linie. Sinds de tweede helft van de 20e eeuw wordt het in grote mate commercieel geproduceerd, waardoor de verschillende soorten niet meer authentiek zijn. Bij Tibetaanse kloosters stonden vaak vier grote wierookbranders (sangkangs) die in de vier belangrijkste richtingen continu wierookgeur verspreidden. Een bekend voorbeeld hiervan is de Jokhangtempel in de Barkhor-buurt in Lhasa. Deze wierookbranders hadden als doel de goden te behagen en de Jokhang te beschermen.
