Die Vereinigung wurde im Jahr 2000 auf einem G8-Treffen begründet, derzeitiger Geschäftsführer ist Richard Feachem. Die Organisation hat bisher Mittel in Höhe von 9,5 Milliarden US-Dollar eingesammelt. Der mit Abstand wichtigste Unterstützer dieser Institution sind die USA, die dem Fund allein zwischen 2002 und 2006 knapp 2 Mrd. US-Dollar überlassen und für 2007 eine weitere Zuwendung von rund 720 Mio. US-Dollar zugesagt haben. Zwischen 2002 und August 2008 genehmigte der Fonds Finanzhilfen in Höhe von 11,4 Milliarden US-Dollar für 136 Staaten. Im Kampf gegen Aids stellt er ein Viertel aller international zur Verfügung gestellten Gelder, im Kampf gegen Tuberkulose zwei Drittel und im Kampf gegen Malaria drei Viertel. Anlässlich der internationalen Aids-Konferenz in Mexiko-Stadt im August 2008 wurde bekannt, dass die Anfrage an Hilfsgeldern dreimal so hoch sei wie in den Vorjahren und derzeit (August 2008) bei 6,4 Milliarden US-Dollar für 97 Staaten liege. Der Fonds setzt sich unter anderem auch dafür ein, dass internationale Patente die Produktion billiger Nachahmerpräparate, sogenannter Generika, nicht länger blockieren.

Cette fondation a pour particularité d’être inscrite au Registre du Commerce de Genève, et donc de répondre à la législation suisse. Ce n’est pas une agence de l’ONU, bien qu’elle travaille en partenariat avec l’OMS et ait son siège à Genève, comme l’OMS.
