A right bundle branch block (RBBB) is a defect in the heart's electrical conduction system. During a right bundle branch block, the right ventricle is not directly activated by impulses travelling through the right bundle branch. The left ventricle however, is still normally activated by the left bundle branch. These impulses are then able to travel through the myocardium of the left ventricle to the right ventricle and depolarise the right ventricle this way.

Die Erregungsleitungsstörung „Rechtsschenkelblock“ verursacht keine Beschwerden und hat keinen eigenen Krankheitswert. Im Unterschied zum Linksschenkelblock besteht auch kein eindeutiger Bezug zu bedeutsamen Herzerkrankungen, im Zusammenhang mit einem Herzinfarkt weist ein RSB allerdings auf eine ungünstige Prognose hin. Als Ursachen eines Rechtsschenkelblocks kommen Durchblutungsstörungen, Entzündungen des Herzens oder Volumenbelastung der rechten Herzkammer, selten auch Vergiftungen in Frage. Häufig jedoch lässt sich keine Ursache feststellen. Eine spezielle Therapie ist nicht erforderlich. Der EKG-Befund „Rechtsschenkelblock“ ist durch Veränderungen des QRS-Komplexes charakterisiert, der im EKG die Aktivierung der Herzkammern abbildet. Diese Veränderungen können in verschiedenen Ausprägungen auftreten. Als „kompletter Rechtsschenkelblock“ weisen sie auf eine Leitungsstörung im rechten Tawara-Schenkel hin, der als Teil des Erregungsleitungssystems die rechte Herzkammer versorgt. Als „inkompletter Rechtsschenkelblock“ hingegen können sie auch bei Herzgesunden ohne krankhafte Bedeutung vorhanden sein.

The QRS complex often shows an extra deflection which reflects the rapid depolarisation of the left ventricle followed by the slower depolarisation of the right ventricle. RBBB usually has pathological cause, although it is seen in healthy individuals.
