Diese beginnt (primär) an den kleinen Gallengängen (biliär), die durch eine Entzündung zerstört werden. Im längeren Verlauf kann die Entzündung auf das gesamte Lebergewebe übergreifen und schließlich zu einer Vernarbung bis hin zur Zirrhose führen. Der Name „Zirrhose“ ist irreführend, da nicht jeder PBC-Patient zum Zeitpunkt der Diagnose bereits eine Zirrhose hat. Der Begriff stammt noch aus Zeiten, in denen die PBC erst im Endstadium entdeckt werden konnte. Inzwischen lässt sich die Erkrankung (z. B. durch den Nachweis von antimitochondrialen Antikörpern, AMA im Blut) bereits nachweisen, wenn die Leber noch relativ unversehrt ist. Daher wird heute auch der Begriff Primär biliäre Cholangitis verwendet.
