Nikola Janakiev Karev (Bulgarian and Macedonian: Никола Карев) was a revolutionary in Ottoman-ruled Macedonia. He was born 23 November 1877 in Kruševo and died 27 April 1905 in the village of Rajčani, both today in the Republic of Macedonia. Karev was a local leader of what later became known as the Internal Macedonian Revolutionary Organization (IMRO). He was also a socialist and a member of the Bulgarian Workers' Social Democratic Party.

Er war Mitglied der Bulgarische Makedonien-Adrianopeler Revolutionären Komitees (kurz BMARK) und der Bulgarischen Arbeiter und Sozialdemokratischen Partei. Während des Ilinden-Preobraschenie-Aufstands war er Vorsitzender der nur vom 3. bis zum 13. August 1903 existenten Republik Kruševo. In dieser Zeit verfasst er gemeinsam mit Nikola Kirow das Manifest von Kruševo. Nach der Niederschlagung des Aufstandes flüchtet er erneut nach Bulgarien. 1905 überquert er die bulgarisch-osmanischen Grenze als Wojwode einer Tscheta und war in Kampfhandlungen mit den osmanischen Miliz verwickelt. Bei einem dieser Gefechten wurde er nahe Rajčani tödlich getroffen.
