It started in Monterrey as Diario de Monterrey, and expanded to other cities in the first decade of the 21st century. During elections, Milenio publishes the acclaimed María de las Heras poll, that was the only poll in Mexico to predict the victory of Vicente Fox in 2000. The news paper also publishes a biweekly magazine distributed nationwide.

De krant geldt als liberaal, maar is minder uitgesproken in haar politieke opvattingen dan de meeste andere Mexicaanse kranten. Voorzitter van de krant is Francisco A. González. Bekende medewerkers zijn Joaquín López-Dóriga, Roberto Hernandez, Jr. , Carlos Marín, Héctor Aguilar Camín, Miguel Cane, Diego Osorno, Carlos Manuel Cruz Meza, Diódoro Carrasco en Ciro Gómez Leyva. In september 2008 begon Milenio een (betaalde) 24-uurs nieuwszender, gericht op het doorbreken van het duopoli Televisa-TV Azteca.
