In 1997 werd hij opgenomen in de a-kern maar speelde geen enkele wedstrijd in de Serie A. Het seizoen daarop werd hij in één seizoen aan twee verschillende clubs uitgeleend, Modena en Varese. Maccarone ging vervolgens bij de Italiaanse derdeklasser AC Prato aan de slag. Later kwam hij via Empoli in Engeland terecht, bij Middlesbrough. Anno 2008 speelt hij bij AC Siena, in de Serie A. Tussen 2000 en 2002 speelde Maccarone voor het Italiaanse U-21 team. In 15 wedstrijden vond hij 11 keer het doel. Voor de nationale ploeg kwam hij twee keer uit, in 2002 tijdens zijn periode in de Premier League.
