His biggest club successes came when he played for PSV Eindhoven in the mid- and late 1970s. After his retirement in the early 1990s he became a football manager, who worked for clubs like F.C. Den Bosch, RBC Roosendaal and Stormvogels Telstar. On 29 May 2008, he was announced as the new Head Coach of English Football League Championship side Southampton as an immediate replacement to Nigel Pearson, with Mark Wotte taking over the Academy. In taking the Southampton job he became the club’s 10th head coach in as many years as well as being the first foreigner to hold the position. But within days it emerged that Poortvliet was still employed by his previous Club Helmond Sport who hadn't given him any permission to talk to Southampton. Poortvliet was offering to pay compensation to Helmond where he still had one year on his contract. But his offer of 30,000 Euros was taken as an insult by Helmond. The situation though was resolved via an arbitration panel (The Dutch FA - KNVB). Jan was told he could take the Southampton F.C. job if he paid his former club Helmond Sport £60,000 (75,000 euros) in compensation. Which he duly agreed to and his contract with his former club Helmond Sport has been terminated. In his first official game in charge Southampton lost 2-1 to Cardiff, having conceded right at the end. However, 3 days later Poortvliet recorded his first win as Southampton manager, beating Exeter City 1-3 in the Carling Cup first round. His first league win came on 23 August at Derby County thanks to a David McGoldrick goal. On 23 January 2009, following a run of just one win in 14 home games, Poortvliet announced his resignation from the Club, with reserve team coach Mark Wotte taking over as head coach. On 15 June 2009, Poortvliet was announced as the new manager of FC Eindhoven.

Met deze club won hij in 1978 de UEFA-cup en in hetzelfde jaar speelde Poortvliet met het Nederlands Elftal op het WK in Argentinië waarbij de finale werd gehaald. In 1983 verliet hij PSV en ging spelen bij Roda JC uit Kerkrade. Hierna speelde hij in het betaald voetbal nog voor Olympique Nîmes, FC Antwerp, AS Cannes, Eendracht Aalst, VC Vlissingen en VCV Zeeland. Poortvliet speelde in totaal 19 interlands waarin hij één keer tot scoren kwam. Poortvliet is onder andere trainer geweest van RBC Roosendaal, FC Den Bosch (waarmee hij in 2001 promoveerde naar de eredivisie) en Stormvogels Telstar. Nadat hij hooftrainer was bij de zondagamateurs van RKSV Nuenen en bij de zaterdagamateurs van vv Arnemuiden werd hij met ingang van het seizoen 2007/08 trainer bij Helmond Sport. Na goede prestaties bij Helmond bood de Engelse Championship-club Southampton FC hem een contract aan, waar Poortvliet eind juni zijn handtekening onder zette. Het avontuur in Engeland werd echter niet wat menigeen ervan verwachtte, Poortvliet incluis. Op 23 januari diende hij zijn ontslag in bij de clubleiding. Dit pakte niet goed uit voor Southampton want Poortvliets opvolger Mark Wotte wist degradatie niet te voorkomen. Vanaf juli 2009 wordt hij hoofdtrainer bij FC Eindhoven.
