Robert Englund reprises his role as Freddy Krueger; Lisa Zane, Yaphet Kotto, Breckin Meyer, Shon Greenblatt, Ricky Dean Logan, Lezlie Deane and Tobe Sexton also star. Additionally, several well-known actors make cameo appearances in the film, including Johnny Depp (whose screen debut was in the original Nightmare On Elm Street), Roseanne Barr, Tom Arnold and Alice Cooper. Iggy Pop sings the movie's title song, which plays over a montage of scenes from the previous Nightmare movies during the end credits.

Spencer, Carlos y Tracy, tras ser descubiertos por la psicóloga, deciden marcharse de allí, pero se pierden, y terminan entrando en una casa abandonada, en la que vivió Freddy. Mientras, Maggie y John, siguen investigando acerca del asesino, ya que adoptan la hipótesis de que el joven es hijo de Krueger, y por eso aún no lo ha matado (A pesar de ser el último). Spencer y Carlos morirán dentro de la casa, mostrándose las muertes en escenas bastante duraderas. Maggie y John llegarán a tiempo para llevarse a Tracy de allí. Pero por el camino de vuelta, muere John. Tracy es acechada en sus sueños, a tarvés de su fallecido padre. Por su parte, la psicóloga descubre en los suyos que, en realidad, es ella misma la hija de Krueger. Se enfrentará a su padre y lo vencerá, liberando los tres demonios del sueño que lo dotan de las habilidades asesinas. El final del filme afirma la supuesta muerte "definitiva" del asesino del guante. Aunque reaparece en dos partes más.
