He was a professor in Physiology in Utrecht from 1847 and was internationally regarded as an authority on eye diseases, directing the Netherlands Hospital for Eye Patients. He was the one of the founders of the science of ophthalmology and is primarily known for introducing prismatic and cylindrical lenses for treatment of astigmatism. Donders also was the first to use differences in human reaction time to infer differences in cognitive processing. This concept is now one of the central tenets of cognitive psychology— while mental chronometry is not a topic in itself, it is one of the most common tools used for making inferences about processes such as learning, memory, and attention.

Seine wissenschaftlichen Beiträge über die Augenbewegungen, über die Verwendung von zylindrischen und prismatischen Gläsern bei Fehlsichtigkeit, über die Akkommodation und die Unterscheidung der Presbyopie von der Hyperopie (Übersichtigkeit und Alterssichtigkeit) waren bahnbrechende Beiträge zur Forschung am Auge. Auch die Korrektur des Astigmatismus durch Zylindergläser geht auf Donders zurück. In der Folge setzte er sich dafür ein, dass die Brillenverordnung von Augenärzten vorgenommen und nicht länger herumreisenden Händlern überlassen wurde. Im Jahr 1863 entwarf er das erste Tonometer, ein Gerät zum Messen des Augeninnendrucks. Zusammen mit Hermann von Helmholtz und Albrecht von Graefe gilt Frans Cornelis Donders als Begründer der modernen Ophthalmologie. Als Resultat seiner physiologischen Arbeiten wird die Pleurahöhle (inzwischen selten) auch als "Donders-Raum", der dort vorhandene Unterdruck (als "intrapleuraler Druck") dementsprechend auch als "Donders-Druck" bezeichnet.
