Später wurde sie Ärztin. 1948 erbte sie £ 500, um ein Hospiz zu eröffnen. Es dauerte aber noch 20 Jahre, bis 1967 das St. Christopher's Hospice in Sydenham im Südosten Londons eröffnet werden konnte. In den 1950er Jahren arbeitete sie sieben Jahre lang als Ärztin in dem Hospiz der Schwestern der Barmherzigkeit. Ihre Überzeugung, dass es möglich ist, die letzten Tage eines Menschen angenehm zu gestalten, trug sie in die Welt. Die Entwicklung von Palliativmedizin und Hospizen sind ganz wesentlich ihr zu danken. Saunders erhielt zahlreiche Auszeichnungen. 1980 wurde sie durch Königin Elizabeth II. als Dame Commander of the Order of the British Empire ausgezeichnet und damit in den persönlichen Adelsstand erhoben. 1989 wurde sie durch Elizabeth II. in den Order of Merit (OM) aufgenommen, ebenfalls 1989 als einzige Frau des 20. Jahrhunderts in England zum Ehrendoktor der Medizin ernannt (übergeben durch den Erzbischof von Canterbury). 2001 erhielt sie den Conrad N. Hilton Humanitarian Prize (2001; dotiert mit 700.000 £). Sie war zweimal verheiratet. Als Christin lehnte sie Lebensverkürzung strikt ab. Sterben war für sie die Chance, Freunden und Familie noch einmal Danke zu sagen. In Großbritannien gibt es nun 220 Hospize und weltweit über 8.000. Sie selbst starb 87jährig in dem von ihr eröffneten Hospiz.

V 39 letech získala doktorát medicíny a stala se světově uznávanou kapacitou v oblasti paliativní medicíny, umírání a lékařské etiky, na tato téma přednášela na řadě univerzit po celém světě. Napsala též řadu knih a pojednání tematicky se věnujícím oblastem jejího díla. Jako hluboce věřící křesťanka odmítala euthanasii a o křesťanství prohlásila, že je a zůstane základním kamenem hospicového hnutí, kamenem, kterým možná není vidět, ale na kterém celá stavba stojí. Za svoji práci v sociální, zdravotnické a náboženské oblasti získala nejvyšší možná ocenění: z rozhodnutí Alžběty II. se stala nejprve důstojnicí a později rytířkou Řádu britského impéria, v roce 1981 obdržela Templetonovu cenu a v roce 2001 též nejprestižnější a nejvyšší ocenění za humanitární dílo: Humanitární cenu Conrada N. Hiltona. Byla (mimo jiné) členkou Royal College of Physicians, Royal College of Nursing a Royal College of Surgeons of England, obdržela desítky čestných doktorátů. Zemřela na rakovinu ve výše zmíněném hospici svatého Kryštofa. Jejím manželem byl polský emigrant malíř Marian Bohusz-Szyszko.
