Les joueurs sont confrontés à des situations où ils doivent déjouer les complots de ces créatures et de leurs adorateurs. Les personnages évoluent dans les années 1890, 1920 ou bien dans un monde contemporain alternatif, mais le principe est en fait utilisable dans quasiment n'importe quelle époque ou lieu. Le système de jeu utilisé — le Basic RolePlaying System — est habituellement considéré comme rapide et simple d'utilisation. Il utilise deux dés à dix faces, permettant de définir les caractéristiques physiques et mentales des personnages ainsi que leurs compétences. Une version D20 System, basée sur un système développé par les créateurs de Dungeons & Dragons, est également disponible. La gamme de suppléments disponible dans le commerce, que ce soit en version originale ou dans sa traduction française, est très riche et comprend de nombreux scénarios et campagnes ainsi que des suppléments de règles et de background. Il s'agit également du jeu de rôle ayant bénéficié du plus grand nombre d'éditions.

Jest oparta na motywach zaczerpniętych z opowiadania o tym samym tytule autorstwa H.P. Lovecrafta oraz jego innych utworów. Zew Cthulhu to gra popularna zarówno na Zachodzie, jak i w Polsce, choć powoli traci na znaczeniu. Standardowo akcja może toczyć się w jednym z trzech okresów: 1890, 1920 i 1990. Zazwyczaj fabuła jest osadzona w drugim z nich, w Stanach Zjednoczonych. Gracze wcielają się w Badaczy Tajemnic - ludzi, którzy stykają się z napawającymi grozą Mitami Cthulhu. Ich celem jest rozwiązywanie mrocznych zagadek, odkrywanie niezwykłych reliktów zapomnianych cywilizacji, badanie bluźnierczych i zakazanych ksiąg oraz powstrzymywanie niebezpiecznych kultów. Większość atrybutów ma wartość procentową, do testów używa się tylko kości k100, jednak inne też mają zastosowanie, choćby do zwiększania umiejętności postaci, odejmowania punktów poczytalności i wyznaczania początkowych cech postaci. Nie ma Punktów Doświadczenia, zamiast nich ulegają zwiększeniu atrybuty. W roku 2001 firma Wizards of the Coast wydała wersję opartą na mechanice d20 - z możliwością zaadoptowania postaci z gry Dungeons and Dragons. Wśród firm tworzących własne rozszerzenia Zew Cthulhu można wymienić Pagan Publishing z Delta Green.
