Alison Forman (born 17 March 1969 in Maitland, New South Wales) is a retired female football (soccer) midfielder from Australia. She captained The Matildas at the 2000 Summer Olympics in Sydney, Australia, where the national team ended up in seventh place. Forman is one of the few Matildas with experience playing as a professional overseas. She played with Danish club Fortuna Hjorring from 1992 to 2005. Along the way she collected honours, including three Danish Championship gold medals.

She played for Northern New South Wales Pride in the Australian League, having made her debut in 1999. She was one of the twelve players who appeared in the Matildas' nude calendar for 2000.
